What if you guys create the same version of this game, but the AI guiding you was an evil entity in disguise. Knowing that you are the only doctor capable of destroying the virus/ bacteria/ sickness/ etc., the evil AI planned to shrink and inject you to an infected human, with a cover-up that you will take part in curing the person, but in reality the AI just wanted to trap you there, in a microscopic world insignificantly smaller than a cell, possibly only as large as a protein, ironically to be killed by something you studied to cure and defeat, or left you to rot inside.
